Xylitol and Cavity Prevention: What the Research Shows
Xylitol has more direct clinical support than most dental-health ingredients — but the evidence is specific about the form and dose that actually work.
What the evidence actually supports
A Cochrane Collaboration review looking at xylitol-containing chewing gum found real, statistically significant preventive effects across 10 studies covering nearly 3,500 participants — with 9 of those 10 studies showing a clinically meaningful reduction in cavities, particularly in children with moderate-to-high baseline risk.
The form matters — gum outperforms candy
This is a detail that's easy to miss: the strong evidence is specifically for xylitol chewing gum. Research on xylitol candies found a significant preventive effect in only one of six studies. The mechanical act of chewing — which increases saliva flow — appears to be part of what makes the gum format effective, not just the xylitol itself.
Dose and mechanism
Research indicates the effective dose is over 5–6 grams of xylitol per day, with at least 5 minutes of chewing time, to meaningfully reduce cavity risk. Mechanistically, xylitol increases saliva flow and reduces levels of Streptococcus mutans (a primary cavity-causing bacterium) by disrupting its energy production, while also raising oral pH in ways that reduce acid production and bacterial adhesion to teeth.
